New Client Onboarding

We look forward to helping you grow your practice.  Onboarding consists of three (3) steps:

Step 1: Complete this form.  It will tell us the case types you want, how many leads per month, where to send to, and the key contact people at your firm.

Step 2: Create your billing profile.  After you complete this onboarding form, you will be redirected to a page where you will create your billing profile.

Step 3: Review and Confirm.  We will review everything and confirm with you the details, when we should be completed, and the estimated campaign start date.


Case Requirements

Below we present the base case qualifiers for each of the mass torts that you selected previously.  If these qualifiers are acceptable to you, select "Yes."  If you would like to customize the requirements, select "No" and a text box will show up that will allow you to customize the specific requirements and qualifying factors for that specific mass tort.  Based on this information, we will provide you pricing based on your requirements.


Roundup:

Camp Lejeune:

  1. Worked or lived at Camp Lejeune between 1953 - 1987
  2. At Camp Lejeune for at least 30 days
  3. Diagnosed with various cancers, Parkinson's, or suffered the loss of a child
  4. Does not currently have an attorney


NEC Formula:

  1. Child diagnosed with NEC as an premature infant
  2. Used cow's milk based baby formula
  3. Does not currently have an attorney


3M Military Earplugs:

  1. Active in military between 2003 - 2015
  2. Wore dual-ended combat arms earplugs while serving in combat
  3. Suffered hearing loss, ringing ears or tinnitus
  4. Does not currently have an attorney


Hernia Mesh:

  2. Had or has scheduled a hernia mesh revision surgery
